An application in writing shall be filed with the clerk for a license under this article.
The application shall set forth the applicant's name, permanent and local address,
business address if any, physical description, recent photograph, right thumb print
and, unless a solicitor, a certificate signed by the county health officer or other
local physician or physician assistant that the applicant is in good health and free
from any contagious diseases. The application shall also set forth the applicant's
employer, if any, and the employer's address, the nature of the applicant's business,
the last three places of the applicant's business and the length of time sought to
be covered by the license. An application fee as set by the council shall be paid
at the time of filing the application to cover the cost of investigating the facts
stated therein.
